Late registrations will be charged an extra $15.00.
 

FOR EACH CAMP ATTENDEE:

1. DOWNLOAD & PRINT the Medical Release Form (click on the red download button on this page)

2. FILL OUT & SIGN

3. NOTARIZE the release form; It MUST include Notary's seal since the camp is out of state.

4. Bring the completed & NOTARIZED form to the registration table at camp on the day of check-in.
